Jennifer is a doctoral-prepared Family Nurse Practitioner who has been working in primary care and geriatrics for over 12 years.

Dr. Paul Gilbert
MD
Dr. Gilbert received his emergency medicine training from University of Illinois at Chicago and has practiced as an ABEM Board certified Emergency Medicine Physician for 20 years.

Kristalle Payano
PA-C
Kristalle was born in the Philippines and grew up in NY/NJ. She received her Bachelor of Science in Biology and her Master of Science in Physician Assistant studies from Seton Hall University in South Orange, NJ.

Aneesha Sullivan
MPAS, PA-C
Aneesha is a Board-Certified Physician Assistant that attended graduate school at the Touro University Nevada College of Osteopathic Medicine where she graduated Summa Cum Laude.

Courtney Zimmerman
MSN, FNP-C
Courtney is from the East Coast and completed her Bachelor of Science in Nursing degree at the University of North Carolina Wilmington. Courtney’s nursing background started in many different specialties but focused primarily on cardiac patients.

John Stathakis
DO
John is board certified in dermatology and has over 58 years of experience in the medical field. He graduated from Philadelphia College of Osteopathic Medicine medical school in 1963.

Eunmee “Amy” Chung
DNP
Amy graduated with both her bachelors and her doctorates in nursing from Loma Linda University, CA. She has 6 years of experience as a registered nurse in ER/Trauma and 3 years of experience as a nurse practitioner in urgent care, primary care, and home health care practices. She especially enjoys in-home primary care as it helps ease tension and stress patients may experience in a clinic setting. Providing healthcare in a comfortable and familiar setting for patients and building the relationship that way is a great aspect of home health she enjoys.

Erin is a certified Adult Gerontology Nurse Practitioner with Team Select. She has been in the medical field for over 15 years having worked in Texas and Arizona.

Diana McCully
FNP-BC
A 30+ year Tucson resident with 20+ years in the healthcare field, Diana started her career at the Tucson Heart Hospital in 1999 and remained in cardiac care until the closure in 2007. At that time, she ventured into home health and hospice service. She also taught nursing students at Pima Community College while she was obtaining her MSN-FNP. She has continued home care, providing Primary care and Hospice services to patients in their place of residence since 2015.

Justin Cooper

Justin Cooper is a Physician Assistant with numerous years experience in family medicine, emergency medicine and pain management. He was trained with the DOD Interservice Physician Assistant Program (IPAP) in San Antonio, TX, with clinical rotations at Fort Hood, TX. He has an extensive background to include working in Ambulance, Fire Departments, as an Army Medic, and more. Justin is born and raised in the Phoenix area where he currently resides with his wife and 3 boys.

Elsa has a masters degree in science of nursing as an Acute Care Nurse Practitioner, board certified. She has been practicing in Arizona for over 6 years. Her areas of specialty are vascular, cosmetic aesthetics, and internal medicine. Her nursing background is cardiovascular intensive health unit (CVICU/ICU) for over six years before furthering her education and becoming a nurse practitioner.
